Sage Primary Care, located in central Casper, has been a recognized Patient Centered Medical Home since 2012, meaning we include patients in every aspect of their care. Our providers specialize in the evaluation, diagnosis and treatment of patients from birth to old age, and we are backed by Wyoming Medical Center’s full network of medical specialists. We offer extended hours, same-day appointments and an on-call provider available 24/7 if you have a pressing medical problem.

Injury and illness rarely wait for your scheduled appointment. That's why Sage Primary Care now offers a walk-in clinic for Sage patients so you can see a provider when you need one, no appointment necessary.
Come to the clinic for sore throats, fevers, sinus or ear infections and other symptoms that need quick medical attention, but don't require a trip to the Emergency Room.
The clinic is open 8 a.m. to 4:30 p.m. Mondays through Fridays. Patients are served on a first come, first served basis.
For New Patients
Once your appointment is scheduled, we will mail or email the patient forms (at right) to you. Please complete these forms carefully. Make sure to list all of your current prescription and over-the-counter medications, specifying the dosage and how often you take them. This information is written on the prescription label. If you need help completing the forms, please arrive 20 minutes early so our staff can assist you. Please bring these completed forms, a photo ID, insurance cards and current medication list to your appointment.
Payment is expected at the time of service. We accept cash, personal checks, VISA, Mastercard and Discover. We are happy to submit medical claims to your insurance provider.
