Depending on if you are a non-hospital employee or an outside vendor, you will need to complete and submit paperwork before you enter our facility. This is required in accordance with the Joint Commission. Please locate your pertinent section below to explore the next steps.
Are you planning to have direct patient contact? Are you planning to participate in a patient care area? If so, you will need to provide the following immunization records:
- Hep B (or attestation)
- TB Test (within last year)
- Varicella (verbal confirmation acceptable)
