Sage Primary Care earns highest level of recognition as a Patient-Centered Medical Home

Sage Primary Care was recently awarded Level 3 Recognition as a Patient-Centered Medical Home.

We are proud to announce that Sage Primary Care has received Level 3 Recognition, the highest possible, as a Patient-Centered Medical Home from the National Committee for Quality Assurance (NCQA).

The recognition is bestowed on primary care practices for “using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships,” according to the committee.

The Patient-Centered Medical Home is a primary care model emphasizing care coordination and communication between patients and their caregivers. Its mission is to improve care, create a patient-centered experience and reduce costs. A clinician-led care team coordinates every patient’s care across the healthcare system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider experiences.

Sage has been working to become a Patient-Centered Medical Home since 2011. It earned Level 2 Recognition in 2012 and has been working on Level 3 status since late 2013.

“We have implemented subtle changes to our system throughout the years including more extensive follow through for patients with specific chronic conditions and offering greater accessibility with extended hours and our forthcoming walk-in clinic,” said Sara Cooper, Sage Primary Care practice manager. “This Patient-Centered Medical Home model is a process for Sage to strive for continuous improvements.”

Standards for the Patient-Centered Medical Home were established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association. One of the model’s tenants is that patients “receive the right care, in the right amount, at the right time.” Clinicians, insurers, purchasers, consumer groups and others know the patient-centered medical home is an alternative to the nation’s costly, fragmented delivery system. Research confirms that medical homes can lead to higher quality and lower costs, and can improve patient and provider experiences of care, according to NCQA.
