For New Patients

Once your appointment for bariatric surgery or other procedure is scheduled, we will mail or email the patient forms (at right) to you. Please complete these forms carefully.

Make sure to list all of your current prescription and over-the-counter medications, specifying the dosage and how often you take them. This information is written on the prescription label. If you need help completing the forms, please arrive 20 minutes early so our staff can assist you. Please bring these completed forms, a photo ID, insurance cards and current medication list to your appointment.

Financial obligation

Commercial insurance

Commercial insurance is insurance you get through your employer or on the exchange, such as Blue Cross Blue Shield. For elective surgeries, such as bariatric surgery, your deductible and out-of-pocket limit must be met BEFORE the procedure.

Once you decide to get the surgery, you will be assigned a patient financial counselor at Wyoming Medical Center. Your counselor will call your insurance company to determine your deductible and out-of-pocket obligations and give you an estimate on how much you will likely have to pay.

Once the surgery is scheduled, the counselor will call your insurance company again to determine how much you will have to pay BEFORE your surgery.

Medicare/Medicaid

Medicare and Medicaid have different criteria for covering bariatric surgery. A representative from Dr. Helling’s office will go over this with you.
